Reserve Booth with the BA-CFM


Below this paragraph there are two different Booth Reservation Forms. Retail Shops, Retail Farmers, Farming Businesses, Mariculture Companies and Hard Goods Vendors use the Business Vendor Form. Aquarist Farmers and Hobbyist Organizations that currently lack a business license or state sellers permit use the Aquarist Farmer Form. These aquarist farmers will need temporary permits from the State of California for this event (see Vendor Home menu item above for details). These forms send the information through normal unsecured email. Please do not include any financial information other then selecting a desired type of payment. Your browser must also be set up for emailing. To test your browser trying sending us a 'testing browser' message by clicking this email address support@ba-cfm.com . Alternatively, you can print the empty form and then write out the information on the blank form boxes. Then mail the completed form to the address listed at the bottom of this page. You can also send an email containing the required information to sales@ba-cfm.com . When the Vendor Booth Application has been approved, vendors will be able to make secure payments through our secured web sales pages located at shop.sw-cfm.com .

Please note that this form will not be sent via secure connection. It will be sent via normal email. Your web browser will need to be configured for sending email for this form to properly send this information. For this reason we are not requesting financial information in this form. We just need basic contact information that should already be available to the public. You can also print the form and mail it to the address at the bottom of this page. If you cannot send the form please contact sales@ba-cfm.com and send the required information via email or request we send an email or fax version of the form. After recieving the completed Business Vendor Booth Reservation Form we will contact you and verify the information. If you want to defer payment (deadline is April 5th 2009) select Reservation in the Payment Type above. If you have selected either Visa, MC or Paypal payment methods we will contact you to request additional billing information. The reservation of a specific booth requires full payment. You can find booth number designations on the Booth Location menu item above. When you have completed the form press the Send button above to transmit the form to the BA-CFM. After being approved vendors can pay via our secure site located at shop.sw-cfm.com . That secure site accepts Visa and MC through our merchant account and also paypal via our paypal account. We can also process visa and mc credit cards manually through our merchant account. You should recieve a response from the BA-CFM within one week.
